An exit slip is a type of formative assessment that you give to students at the end of a lesson. Typically, they are brief and usually only assess a small skill set.
As teachers, we can use exit slips to help guide and inform instruction. You can easily assess a student’s understanding of a topic and whether or not more instruction is needed individually or as a whole class.
If the majority of students succeed, you know that you’ve properly taught the skill and you can move on to new material or spend only a brief amount of time reviewing it. Should only a small portion of students perform poorly, you then know who to pull into small groups for additional support. If all or most of the students perform poorly, you know immediately that you need to re-teach or alter your approach to instruction.
While the picture below shares an exit slip I used with my own children to show their ELA and math skills, it could easily be altered to only assess one or the other.
One thing that is helpful on this exit slip template is the picture guides. In the corner of each box, you’ll note a small black icon. This makes it very easy to give verbal directions so that you can assess a large group.
For example, the teacher could say:
Put your finger on the dog. In the box with the dog, draw a face.
The picture guide is especially helpful for non- or emergent readers.
